ADVANCED PROVIDER OFFICE ROLE
Shipley Cardiothoracic Center – Added Advanced Provider Positions to the office setting.
As a center of excellence, we strive to ensure the best outcome for each patient. Many of our patient population have complex medical problems. We know that we can provide great care through holistic patient assessment. A patient’s risk for surgery is defined by potentially modifiable factors. Advanced Providers play a vital role in reviewing the patients’ medical history and current clinical condition. Many of our patients benefit from optimization of other chronic illnesses prior to undergoing cardiac or thoracic surgery. Common areas of optimization include cardiovascular, respiratory, renal, anemia, endocrine, malnutrition, infectious processes, cognitive dysfunction, frailty, and even emotional readiness. Optimization for CT surgery patients occur at the inpatient and outpatient phases of care. Attention to detail makes a tremendous difference in the patient experience. Each patient has unique medical, emotional, and psychosocial elements to their current condition that makes a great impact on the overall care provided in terms of patient safety. APP Outpatient Role
Evaluate each office appointment in advance to ensure readiness for complete MD evaluation order tests, peer-peer PRN for OP insurance authorization.
Shared office visit -AP & MD – Assessment, Evaluation, & Treatment 15-20 patients 4 days/wk
APP patient appointments to update history and physicals prior to surgery if seen > 30 days ago
Case presentation & imaging evaluation with surgeon for each patient appointment
Optimization Coordination w/other specialties (Pulm, Heme/One, Nephrology, Cardiology, GI)
Patient Education
Care Management
Resource to support staff
Order management pre office visit and pre-surgery orders
Results review/collaboration with MD on lab, radiographic, EKG, CT Scans, Echo, Cath etc
Management of Aneurysm Recall Program in collaboration with surgeons-review surveillance imaging, determine & document ongoing plan for support staff
Available for immediate response to telephone inquiries requiring APP input (from patient, referring MD’s, or ancillary departments).
